When you'll be asked for some personal details

To make an appointment with us, you'll be asked for some personal information, like:
- Your first and last name
- Your date of birth
- Your contact number
This is so we can contact you and work out how to help you.
When you visit one of our clinics for the first time, the medical receptionist will give you a Client Registration Form to fill out and return to the front desk.
You can print and fill out a Client Registration Form before your appointment if that makes things easier for you.
